Cuse only said it's not playing out in someone's head, not that it wasn't virtual. I've had a theory for awhile that we haven't seen the original crash. All of the Losties were in a plane crash together before the first one we see - and it created major survival guilt issues for them. I think in the original one a bomb went off, and the survivors are feeling guilt, anger, etc. Their playing a virtual reality game - "a very unique, special therapy" to get them fixed. But, the only way they'll succeed is if they work through it together. I do think they are in a mental institution on an island playing it, and the flash backs, forewards, etc. are a second part of that game.As, as bizarre as it sounds - SMOKEY actually could represents the evil crash they had, and their fight to overcome it. Just look at all the strange plane, bomb, crash, mental institution, psychological testing and game references. And look at all the crashes that happen throughout the off-island looks - they're just reliving things through the game until they're fixed.

"Just to add on to the theory that it's a psychological game, in re-watching some of the shows I noticed a bunch of "baggage" issues in the sideways flashes, particulalry LA X (Artzt luggage gets run over, Locke loses his luggage w/ knives (or maybe his anger baggage), Claires "baby baggage/luggage" gets taken by Kate, Jacks Dad (or "daddy issues" luggage gets lost. That made me go back and randomly start watching other shows to see when folks were carrying backpacks. Now, this is strange, but I really think that when they are carrying backpacks - they are literally carrying their "psychological baggage on their back. And when they carry guns they're carrying their anger. Remember Locke handing Jack the backpack w/ the bomb - I think he literally gave him the baggage that had to be dealt with." Baggage? Claire ish confused >.<

The name DHARMA? The definition of DHARMA is "One's Righteous Path in life"
Now, it may well turn out to be a company LOGO on some surgical machine inside the operating room. Such as: This machine Manufactured by DHARMA INC, a Division of Whitmore Industries. A classic Writer's tactic.
But more likely, one should consider the following:
This whole story is, in summation, the Walk-A-Bout that John Locke needed in real life, that he was robbed of in Australia.
It's just happening in his coma and the ramifications are clearly more eye-opening for him than the real thing would have been. If John Locke survives the surgery, he will be healed, happy and more importantly, finally in touch with his real self.
Which is the real purpose of a Walk-about!
It would be John's Dharma!
John's Righteous Path!

I'm such a fan of Lost....I just hope that it's NOT in any particluar character's mind. I want it to be real. And what's the deal with Jack and Kate? Did they know each other before or was this a preview into the future?

After Feb. 14th's episode I'm beginning to think it's all in Desmond's head. Follow me here...he was shipwrecked on the island, alone when the plane crashes, rushes to the crash site, he is trying to save the passengers but realizes he can't change the outcome. They're going to die, if not already. He goes back in his head giving them lives and stories trying to get a different scenerio. This thing better not end with Desmond standing over a bunch of dead passengers at the time of the crash.

The numbers were related to the hatch that Locke found. They related to the computer that Desmond used. They were given out of the mayday call, by the french voice (Danielle and man's voice). They were carved into Ekos staff. I think these numbers have a connection to several of the losties, Hurley just got hit hardest with them. I'm willing to bet that these numbers intertwined with more of the losties and maybe the others and I'm sure we have not heard the last of them
