Not much is known about the incident. It was first mentioned in the Swan Orientation Video. Dr. Marvin Candle said that there was an "Incident", soon after experiments began in the Swan, which was originally designed to study electromagnetism. A protocol, which is the computer, was created after the incident to stop the growth of the electromagnetic anomaly. The code had to be typed every 108 minutes to stop this from happening.

It is unknown how or when the incident happened. It did happen before 1980, because the production date of the Orientation Film was 1980. Failure to enter the code will result in a system failure. A failsafe key can be used to stop the electromagnetic anomaly, but destroying the Swan, which happened when Desmond turned the key.
